Januvia (Sitagliptin Phosphate Tablets) is used as an adjunct to diet and exercise to improve glycaemic control in adult patients with type 2 diabetes mellitus.
This medicine is used as monotherapy in patients inadequately controlled by diet and exercise alone and for whom Metformin is inappropriate due to contraindications or intolerance. It is used as dual oral therapy in combination with Metformin when diet and exercise plus Metformin alone do not provide adequate glycaemic control. Also used with a sulphonylureaas dual oral therapy when diet and exercise plus maximal tolerated dose of a sulphonylurea alone do not provide adequate glycaemic control and when Metformin is inappropriate due to contraindications or intolerance.

This medicine can also be used as triple oral therapy in combination with a Sulphonylurea and Metformin when diet and exercise plus dual therapy with these medicinal products do not provide adequate glycaemic control.

Januvia tablets are manufactured by Merck Sharp and Dohme in the strengths of 50 mg and 100 mg. What Are Januvia (Sitagliptin Phosphate Tablets) Tablets Used For?

Januvia (Sitagliptin Phosphate Tablets) is a prescription medicine used along with diet and exercise alone or in combination with certain other medicines (metformin, ertugliflozin, medicines such as rosiglitazone and pioglitazone, a sulfonylurea medicine such asglimepiride, gliclazide and glibenclamide, or insulin), to lower blood sugar in adult patients with type 2 diabetes when diet plus exercise or the other medicine(s) do not provide adequate blood sugar level control.
Januvia lowers blood sugar when blood sugar is high, especially after a meal. Januvia also lowers blood sugar between meals. Januvia helps to improve the levels of insulin produced by your own body after a meal and decreases the amount of sugar made by the body.

Type 2 diabetes mellitus is a condition in which your body does not make enough insulin and the insulin that your body produces does not work as well as it should. Your body can also make too much sugar. When this happens, sugar (glucose) builds upin the blood. This can lead to serious medical problems.
The main goal of treating diabetes is to lower your blood sugar to a normal level. Lowering and controlling blood sugar may help prevent or delay complications of diabetes, such as heart disease, kidney disease, blindness and amputation. High blood sugar can be lowered by diet and exercise and by certain medicines.
Januvia is unlikely to cause your blood sugar to be lowered to a dangerous level (hypoglycemia) because it does not work when your blood sugar is low. Januvia has not been studied in children under 18 years of age.

Januvia (Sitagliptin Phosphate Tablets) - Dosage

The recommended dose of Januvia (Sitagliptin Phosphate Tablets) is 100 mg once daily. Januvia tblets can be taken with or without food. Dosage adjustment is recommended for patients with moderate or severe renal insufficiency or end-stage renal disease.

Januvia (Sitagliptin Phosphate Tablets) Contraindications

Januvia tablets are contraindicated in patients with a hypersensitivity to Sitagliptin Phosphate or any inactive ingredient present in tablets. Januvia tablets are also contraindicated in the following cases:
Patients with severe renal impairment(eGFR below 30 mL/min/1.73m2)
Acute or chronic metabolic acidosis, including diabetic ketoacidosis. Diabetic ketoacidosis should be treated with insulin.
Januvia has not been studied in patients with a history of pancreatitis.

Warnings and Precautions

 

  • There have been postmarketing reports of acute pancreatitis, including fatal and non-fatal hemorrhagic or necrotizing pancreatitis. If pancreatitis is suspected, promptly discontinue Januvia tablets.
  • There have been postmarketing reports of acute renal failure, sometimes requiring dialysis. Dosage adjustment is recommended in patients with moderate or severe renal insufficiency and in patients with ESRD. Assessment of renal function is recommended prior to initiating Januvia tablets and periodically thereafter.
  • There is an increased risk of hypoglycemia when Januvia tablets is added to an insulin secretagogue (e.g., sulfonylurea) or insulin therapy. Consider lowering the dose of the sulfonylurea or insulin to reduce the risk of hypoglycemia.
  • There have been postmarketing reports of serious allergic and hypersensitivity reactions in patients treated with Januvia tablets such as anaphylaxis, angioedema, and exfoliative skin conditions including Stevens-Johnson syndrome. In such cases, promptly stop Januvia tablets, assess for other potential causes, institute appropriate monitoring and treatment, and initiate alternative treatment for diabetes.
  • There have been no clinical studies establishing conclusive evidence of macrovascular risk reduction with Januvia tablets or any other anti-diabetic drug.

 

Januvia (Sitagliptin Phosphate Tablets) Side Effects

Side effects reported in 5% or more of patients treated with Januvia (Sitagliptin Phosphate 50 mg and 100 mg tablets) and more commonly than in patients treated with placebo are: upper respiratory tract infection, nasopharyngitis and headache. In the add-on to sulfonylurea and add-on to insulin studies, hypoglycemia was also more commonly reported in patients treated with Januvia tablets compared to placebo.

Januvia (Sitagliptin Phosphate Tablets) Overdosage

During controlled clinical trials of Sitagliptin Phosphate (Januvia tablets) in healthy subjects, single doses of up to 800 mg Januvia were administered. Maximal mean increases in QTc of 8.0 msec were observed in one study at a dose of 800 mg Januvia, a mean effect that is not considered clinically important. There is no experience with doses above 800 mg in clinical studies. In Phase I multiple-dose studies, there were no dose-related clinical adverse reactions observed with Januvia with doses of up to 600 mg per day for periods of up to 10 days and 400 mg per day for up to 28 days.

In the case of an overdose, it is reasonable to employ the usual supportive measures, e.g., remove unabsorbed material from the gastrointestinal tract, employ clinical monitoring (including obtaining an electrocardiogram), and institute supportive therapy as dictated by the patient's clinical status. Sitagliptin is modestly dialyzable.
In clinical studies, approximately 13.5% of the dose was removed over a 3- to 4-hour hemodialysis session. Prolonged hemodialysis may be considered if clinically appropriate. It is not known if Sitagliptin is dialyzable by peritoneal dialysis.

Januvia (Sitagliptin Phosphate Tablets) During Pregnancy

Januvia (Sitagliptin Phosphate tablets) has been classified by the US FDA as Pregnancy Category B. Reproduction studies have been performed in rats and rabbits. Doses of sitagliptin up to 125 mg/kg (approximately 12 times the human exposure at the maximum recommended human dose) did not impair fertility or harm the fetus.

There are, however, no adequate and well-controlled studies in pregnant women. Since animal reproduction studies are not always predictive of human response, this medication should be used during pregnancy only if clearly needed. Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Sitagliptin Phosphate (Januvia Tablets) Nursing Considerations

Sitagliptin (Januvia tablets) is secreted in the milk of lactating rats at a milk to plasma ratio of 4:1. It is not known whether Sitagliptin is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when Januvia tablets are administered to a nursing woman.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
