Idiopathic pulmonary fibrosis (IPF) is a chronic irreversible and ultimately fatal disease characterized by a progressive worsening in lung function. The term pulmonary fibrosis means scarring of lung tissue and is the cause of worsening dyspnea (shortness of breath).

In most cases, however, the cause of lung scarring is not known and are called Idiopathic Pulmonary Fibrosis (IPF). With IPF, doctors speculate that something inside or outside of the lungs attacks them again and again over time. These attacks injure the lungs and scar the tissue inside and between the air sacs. Things that can increase the risk of pulmonary fibrosis include: Cigarette smoking, Certain viral infections, Exposure to environmental pollutants, including silica and hard metal dusts, bacteria and animal proteins, and gases and fumes.

Active Ingredient present in Pirfenex Tablets

The active ingredient present in Pirfenex Tablets is Pirfenidone. Each film-coated tablet contains 200 mg of Pirfenidone.

Uses of Pirfenex Tablets

Pirfenex Tablets (Pirfenidone) is a pyridine molecule with anti-inflammatory and antifibrotic activities which is used to treat pulmonary fibrosis and liver fibrosis.

Generic Pirfenidone Tablets (Pirfenex) - Dosage

The initial dose of Generic Pirfenidone Tablets (Pirfenex) for adults is 200 mg, three times a day (600 mg/day), after a meal. Gradually increase the dose to 600 mg, three times a day (1,800 mg/day), under observation . Furthermore, appropriately increase or decrease the dose from time to time depending upon the symptoms.

The treatment has to be started with 200 mg tablets given three times a day (600 mg/day). After 2 weeks, the dose has to be gradually increased by 200 mg at a time. It is desirable to maintain or achieve a final dose of 600 mg at a time (1,800 mg/day).

Patients who miss their dose for 14 consecutive days or more of pirfenidone treatment should re-initiate therapy by undergoing the initial dose titration regimen up to the recommended daily dose.
In case the treatment interruption is less than 14 consecutive days, the dose can be resumed at the previous recommended daily dose without titration.

Store Pirfenex at controlled room temperature (59°F to 86°F, 15°C to 30°C). Protect from light. Store in a cool dry place

Contraindications of Pirfenex Tablets

Pirfenex tablets are contraindicated in patients with a hypersensitivity to Pirfenidone or any inactive ingredient present in tablets. Pirfenex tablets are also contraindicated in the following cases:

History of angioedema with Pirfenidone
Concomitant use of Fluvoxamine
Severe hepatic impairment or end stage liver disease
Severe renal impairment (CrCl <30 ml/min) or end stage renal disease requiring dialysis

Warnings and Precautions When Using Generic Pirfenidone Tablets

Elevated liver enzymes: ALT, AST, and bilirubin elevations have occurred with Generic Pirfenidone tablets (Pirfenex). Monitor ALT, AST, and bilirubin before and during treatment. Temporary dosage reductions or discontinuations may be necessary.

Photosensitivity and Rash: Photosensitivity and rash have been observed with Pirfenex tablets. Avoid exposure to sunlight and sunlamps. Wear sunscreen and protective clothing daily. Temporary dosage reductions or discontinuations may be required.

Gastrointestinal Disorders: Nausea, vomiting, diarrhea, dyspepsia, gastro-esophageal reflux disease, and abdominal pain have occurred with Generic Pirfenidone tablets. Temporary dosage reductions or discontinuations may be necessary.

Consumption Of Grapefruit Juice: Consumption of grapefruit juice is associated with inhibition of CYP1A2 and should be avoided during treatment with pirfenidone. Strong CYP1A2 inhibitors should, therefore, be used with caution in patients receiving Generic Pirfenidone due to the potential for reduced clearance.

Smoking: Smoking has the potential to induce hepatic enzyme production and thus increase medicinal product clearance and decrease exposure. Concurrent use of strong inducers of CYP1A2 including smoking must be avoided during Pirfenidone therapy based on the observed relationship between cigarette smoking and its potential to induce CYP1A2. Patients should be encouraged to stop use of strong inducers of CYP1A2 and to discontinue smoking before and during treatment with pirfenidone.

Angioedema: Reports of angioedema (some serious) such as swelling of the face, lips and/or tongue which can be associated with difficulty breathing or wheezing have been received in association with use of Pirfenidone in the post-marketing setting. Therefore, patients who develop signs or symptoms of angioedemafollowing administration of Pirfenidone should immediately stop treatment with this medicine.

Dizziness: Dizziness has been reported in patients undergoing therapy with Pirfenidone. Hence, patients should know how they react to this medicinal product before they engage in activities requiring mental alertness or coordination

Fatigue: Fatigue has been reported in patients taking pirfenidone. Therefore, patients should know how they react to this medicinal product before they engage in activities requiring mental alertness or coordination.

Weight Loss: Weight loss has been reported in patients treated with Pirfenidone. Doctors must monitor patients’ weight, and when appropriate encourage increased caloric intake if weight loss is considered to be of clinical significance.

Ability To Drive And Use Machines: Drowsiness, dizziness and fatigue may occur due to Pirfenidone, which may cause the patient to stagger. No studies on the effects of the ability to drive and use machines have been performed. Patients on Pirfenidone tablets must, therefore, be advised not to operate or drive any machinery or motor vehicles.

Renal Impairment: There is limited experience with Pirfenidone in patients with renal impairment.

Hepatic Impairment: In patients with moderate hepatic impairment (i.e. Child-Pugh Class B), Pirfenidone exposure was increased by 60%. Pirfenidone should be used with caution in patients with pre-existing mild to moderate hepatic impairment (i.e. Child-Pugh Class A and B) given the potential for increased Pirfenidone exposure.

Elderly Patients: Elderly patients generally have declined physiological function; and therefore Pirfenidone therefore must be administered with caution.

Pirfenidone (Pirfenex) Side Effects

The most common Pirfenidone Tablets (Pirfenex) side effects with an incidence rate of 10% or more are nausea, rash, abdominal pain, upper respiratory tract infection, diarrhea, fatigue, headache, dyspepsia, dizziness, vomiting, anorexia, gastro-esophageal reflux disease, sinusitis, insomnia, weight decreased, and arthralgia.

In case any of the following side effects occur while taking Pirfenidone 200 mg tablets , inform your doctor immediately:
More common: Diarrhea, itching or skin rash, nausea, redness or other discoloration of the skin, severe sunburn, stomach discomfort, upset, or pain and vomiting.

Incidence not known: Cough or hoarseness, fever with or without chills, general feeling of tiredness or weakness, large, hive-like swelling on the face, eyelids, lips, tongue, throat, hands, legs, feet, or sex organs, lower back or side pain, painful or difficult urination, sore throat, sores, ulcers, or white spots on the lips or in the mouth and unusual bleeding or bruising

Pirfenidone Overdosage

Inadequate information regarding Pirfenidone Tablets (Pirfenex) overdose is available. Multiple doses of Pirfenidone up to a dose of 4806 mg/day were administered as six 267 mg capsules three times daily to healthy adult volunteers over a 12-day dose escalation period. Adverse reactions were mild, transient, and consistent with the most frequently reported adverse reactions for Pirfenidone.

In the event of a suspected overdose, supportive medical care must be provided including monitoring of vital signs and close observation of the clinical status of the patient.

Pirfenidone During Pregnancy

Pirfenidone has been classified by the US FDA as Pregnancy Category C. Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may justify use of the drug in pregnant women despite potential risks.

Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

Nursing mothers receiving treatment with Pirfenidone tablets must be advised to avoid breastfeeding. It is unknown whether Pirfenidone or its metabolites are excreted in human milk. A decision must be made whether to stop breast-feeding or to discontinue from Pirfenidone therapy, taking into account the benefit of breast-feeding for the child and the benefit of Pirfenidone therapy for the mother.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
