Menotrophin (HMG 75 IU and 150 IU Injection) is used along with Human Chorionic Gonadotropin, for stimulation of ovulation and pregnancy in women with ovulatory dysfunction not due to primary ovarian failure. Menotropins are also used along with Human Chorionic Gonadotropin, for stimulation of spermatogenesis in men with primary or secondary hypogonadotropic hypogonadism. Menotrophin Injection (Menodac) is also used off-label in bodybuilding during Post Cycle Therapy.

Menodac (Menotropins for injection) are manufactured by Bayer Zydus Pharma Ltd. in the strengths of 75 IU and 150 IU. HMG injection costs only for 150 IU strength. Active Ingredient present in Menodac Injection

The active ingredient present in Menodac injection vial is highly purified Menotrophin (Human Menopausal Gonadotrophin, HMG).

Each Menodac 75 vial with dry powder contains highly purified Menotrophin (Human Menopausal Gonadotrophin, HMG) corresponding to 75 IU human follicle stimulating hormone (FSH) and 75 IU human luteinizing hormone (LH).
Each Menodac 150 vial with dry powder contains highly purified Menotrophin (HMG) corresponding to 150 IU FSH and 150 IU LH.

Uses of Menodac Injection (hMG, Human Menopausal Gonadotrophin)

Menotrophin (hMG, Human Menopausal Gonadotrophin) is described in both the British Pharmacopoeia (BP) as well as the United States Pharmacopeia (USP). Highly purified Menotrophin (Human Menopausal Gonadotrophin, HMG) substance is obtained from the urine of menopausal/postmenopausal women.

Infertility, female (treatment)—Menotropins are indicated, in conjunction with chorionic gonadotropin, for stimulation of ovulation and pregnancy in patients with ovulatory dysfunction not due to primary ovarian failure. In general, menotropins are the treatment of choice for induction of ovulation in patients with hypothalamic hypogonadism or those who do not respond to clomiphene.

Menodac Injection is a gonadotropin which can be used to stimulate follicle development in amenorrhoeic patients. In general, menotropins are the preferred treatment for induction of ovulation in patients with hypothalamic hypogonadism or those who do not respond to Clomiphene.

Menodac is also used for the development of multiple follicles and pregnancy in ovulatory women as part of an Assisted Reproductive Technology (ART) cycle. Menodac Injection could also be administered in combination with Human Chorionic Gonadotrophin (e.g. Vitagon, Fertigyn, HUCOG, Eutrig) for the stimulation of spermatogenesis.

HMG In Bodybuilding - Why is Human Menopausal Gonadotropin used in Post Cycle Therapy

Post Cycle Therapy or PCT is a necessity for bodybuilders after they have completed a cycle. Inorder to keep the strength and muscle gains you worked so hard for when you were on the special medications to enhance the muscles, you need to help return your own body's hormonal levels back to normal.

The difference between HCG and HMG is that HCG mimics LH (luetinized hormone) and fools the body into believing that LH is present and everything else behaves normally. HMG actually elevates natural LH, and hence there is a lesser chance of building up a tolerance when this happens.

One big difference between the two is that HMG raises FSH (Follicle Stimulating Hormone) and HCG does not. This is especially attractive since an increase in FSH translates into higher sperm count and ejaculate volume.

Menodac Injection - Dosage

The dosage of Menodac (Menotrophin Injection) for various indications is given below:
Treatment of Anovulatory Infertility:
The dosage and schedule of therapy must be determined according to the requirements of each patient. The response is monitored by studying the patient's urinary oestrogen excretion or by ultrasound visualization of follicles. Menotrophin may be administered daily by either intramuscular or subcutaneous injection to provide a dose of 75 to 150 units of FSH and 75 to 150 units of LH, and gradually adjusted if necessary until an adequate response is achieved, followed after 1 or 2 days by Chorionic Gonadotrophin. In menstruating patients, treatment should be started within the first 7 days of the menstrual cycle. The treatment course should be abandoned if no response is seen in 3 weeks.

Women Undergoing Superovulation in IVF or Other Assisted Conception Techniques: In in-vitro fertilization procedures or other assisted conception techniques, Menotrophin is used along with chorionic gonadotrophin and sometimes also Clomiphene Citrate or a gonadorelin agonist. Stimulation of follicular growth is produced by menotrophin in a dose providing 75 to 300 units of FSH with 75 to 300 units of LH daily. Treatment with Menotrophin injection, either alone or in conjunction with Clomiphene or a Gonadorelin agonist, is continued until an adequate response is obtained and the final injection of Menotrophin is followed 1 or 2 days later with up to 10000 units of chorionic gonadotrophin.

Treatment of Male infertility: Spermatogenesis is stimulated with Human Chorionic Gonadotrophin (1000 IU to 2000 IU two to three times a week) and then Menotrophin (Menodac injection) is given in a dose of 75 units or 150 units of FSH with 75 or 150 units of LH two or three times weekly. Treatment must be continued for at least 3 or 4 months.

Dosage of HMG Injection In Bodybuilding

Leading bodybuilders recommend a very low dose of HMG Injection. A single Menotrophin injection of 75 IU is the preferred choice. Bodybuilders may even split the contents of Menodac 75 IU vial into two half shots over two days. Please consult your healthcare provider for exact dosing information.

Menotrophin (HMG Injection) - Mechanism of Action

Menotropins contain follicle-stimulating hormone (FSH) and luteinizing hormone (LH).
For induction of ovulation and assisted reproductive technologies (ART):
Menotropins prepare the ovarian follicle for ovulation. The combination of FSH and LH stimulates follicular growth and maturation. Chorionic Gonadotropin, whose actions are nearly identical to those of LH, is administered following menotropins treatment to mimic the naturally occurring surge of LH that triggers ovulation.

For treatment of male infertility: Following administration of chorionic gonadotropin to increase testosterone concentrations in men with hypogonadotropic hypogonadism, administration of menotropins induces spermatogenesis

Menodac Storage

Store Menodac injections preferably at controlled room temperature (59°F to 86°F, 15°C to 30°C). Protect from light.

Menodac (HMG Injection) Contraindications

Menodac (HMG Injection) is contraindicated in patients with a hypersensitivity to Menotrophin or any inactive ingredient present in tablets and in patients with tumours of the pituitary or hypothalamic glands. Menodac Injections are also contraindicated in the following cases:
Men:

  • Tumours in the testes
  • Prostate carcinoma

Women:

  • Ovarian, uterine or mammary carcinoma
  • Pregnancy and lactation
  • Gynaecological haemorrhage of unknown aetiology
  • Ovarian cysts or enlarged ovaries not due to polycystic ovarian disease.

In the following situations treatment outcome is unlikely to be favourable, and therefore Menodac (HMG Injection) must not be administered:

  • Primary ovarian failure
  • Malformation of sexual organs incompatible with pregnancy
  • Fibroid tumours of the uterus incompatible with pregnancy
  • Structural abnormalities in which a satisfactory outcome cannot be expected, for example, tubal occlusion (unless superovulation is to be induced for IVF), ovarian dysgenesis, absent uterus or premature menopause.

 

Menodac (Menotrophin Injection) - Warnings and Precautions

Menodac (Menotrophin Injection) is a potent gonadotropic substance capable of causing mild to severe adverse reactions, and should only be used by doctors who are thoroughly familiar with infertility problems and their management.

In the treatment of female infertility, ovarian activity must be checked (by ultrasound and plasma 17 beta-oestradiol measurement) prior to Menodac administration. During treatment, these tests and urinary oestrogen measurement should be carried out at regular intervals, until stimulation occurs.

Ovarian Hyperstimulation Syndrome (OHSS) is a syndrome that can reveal itself with increasing degrees of severity. It consists of marked ovarian enlargement, high serum sex steroids, and an increase in vascular permeability which can result in an accumulation of fluid in the peritoneal, pleural and rarely, in the pericardial cavities.
In case during ultrasound, several mature follicles are observed, Human Chorionic Gonadotrophin should not be administered as there is a risk of multiple ovulation and the occurrence of hyperstimulation syndrome.
Adherence to recommended Menodac dosage, regimen of administration and careful monitoring of treatment will minimise the incidence of ovarian hyperstimulation and multiple pregnancy.In case severe OHSS occurs, gonadotrophin treatment should be stopped if still ongoing, the patient hospitalised and specific therapy for OHSS started.

Multiple pregnancies, especially high order, carries an increased risk of adverse maternal and perinatal outcomes.The incidence of pregnancy wastage by miscarriage or abortion is higher in patients undergoing stimulation of follicular growth for ART procedures than in the normal population.

Women with a history of tubal disease are at risk of ectopic pregnancy, whether the pregnancy is obtained by spontaneous conception or with fertility treatment. The prevalence of ectopic pregnancy after IVF has been reported to be 2 to 5%, as compared to 1 to 1.5% in the general population

There have been reports of ovarian and other reproductive system neoplasms, both benign and malignant, in women who have undergone multiple drug regimens for infertility treatment.
The prevalence of congenital malformations after ART may be slightly higher than after spontaneous conceptions. In women with generally recognized risk factors for thromboembolic events, such as personal or family history, treatment with gonadotrophins may further increase the risk.

Menodac (Menotrophin Injection) Side Effects

The most common Menotrophin injection side effects with an incidence rate between 2% and 7% reported during treatment with this medicine during clinical trials are ovarian hyperstimulation, abdominal pain, headache, enlarged abdomen, inflammation at the injection site, pain at the injection site and nausea.

The common Menodac (Menotrophin Injection) side effects with an incidence rate of more than 1% and less than 10% include Headache, Abdominal pain, enlarged abdomen, nausea and vomiting, Ovarian hyperstimulation, Inflammation at the injection site, pain at injection site and Deep vein thrombosis.

In very rare cases, long term use of Menotrophin can lead to the formation of antibodies making treatment ineffectual.
Very rare cases of allergic reactions, localised or generalised, and delayed-type hypersensitivity have been reported after treatment with Gonadotrophin containing products.

Menotrophin Injection Overdosage

The acute toxicity of Menotrophin Injection (Menodac) has been demonstrated to be very low. However, too high a dosage for more than one day could lead to hyperstimulation, which is categorized as mild, moderate or severe. Symptoms of overdosage usually appear 3 to 6 days after treatment with human chorionic gonadotrophin.

Therapy consists of hospitalization; treatment must be conservative and concentrate on restoring blood volume and preventing shock. Acute symptoms subside over several days and ovaries return to normal over 20 to 40 days if conception does not occur. The Menodac overdose symptoms could be prolonged if conception occurs.

Menotrophin Injection During Pregnancy

Menotrophin Injection has been classified by the US FDA as Pregnancy Category X. Although problems in humans have not been documented, use of Menotropins during pregnancy is unnecessary. Ovarian hyperstimulation syndrome (OHS), which may be induced by menotropins therapy, is more common, more severe, and protracted in patients who conceive. Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

It is not known whether this drug is excreted in human milk. Because many drugs are excreted in human milk and because of the potential for serious adverse reactions in the nursing infant from Menotrophin, a decision should be made whether to discontinue nursing or to discontinue the medicine, taking into account the importance of the medicine to the mother.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
