The active ingredients present in Sartel-H tablets are Telmisartan and Hydrochlorothiazide. Each tablet of Sartel-H contains 40 mg of Telmisartan and 12.5 mg of Hydrochlorothiazide.

Uses of Generic Micardis HCT (Sartel-H by Intas)

This medicine is used for treating high blood pressure. It may also be used for other conditions as determined by your doctor. The medicine is a combination angiotensin II receptor blocker and diuretic. The angiotensin II receptor blocker works by relaxing the blood vessels. Exactly how the diuretic works to decrease blood pressure is not known, but it helps the kidneys to eliminate fluid and sodium from the body.

Telmisartan HCTZ (Generic Micardis HCT) - Dosage

Telmisartan HCTZ tablets(Generic Micardis HCT) may be administered with or without food. The normal starting dose of Telmisartan is 40 mg once a day and the blood pressure response is dose related over the range of 20 mg to 80 mg.
Patients with depletion of intravascular volume should have the condition corrected otherwise the treatment with Telmisartan tablets should be started under close medical supervision. Hydrochlorothiazide is effective in doses of 12.5 mg to 50 mg once daily. To reduce the possibility of dose-independent side effects, it is usually recommended to begin combination therapy only after a patient has failed to achieve the desired effect with monotherapy.

How to Use Generic Micardis HCT (Sartel-H by Intas)

Use Generic Micardis HCT (Sartel-H 40 mg tablets) as directed by your doctor. Take it by mouth with or without food. Do not remove the tablet from the blister until you are ready to take it. Drinking extra fluids while you are taking this medicine is recommended.
This medicine may increase the amount of urine or cause you to urinate more often when you first start taking it. To keep this from disturbing your sleep, try to take your dose before 6 pm.

If you take cholestyramine or colestipol, ask your doctor how to take it with this medicine. Continue to take this medicine even if you feel well. Do not miss any doses. If you miss a dose of this medicine, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Telmisartan HCTZ Side Effects

All medicines may cause side effects, but many people have no or minor side effects. The common side effects occurring in 2% or more of patients undergoing treatment with Telmisartan HCTZ tablets are Fatigue, Influenza-like symptoms, Dizziness, Diarrhea, Nausea, Sinusitis and Upper respiratory tract infection.
Check with your doctor if any of these common side effects of Sartel-H tablets persist or become bothersome: back pain, diarrhea, dizziness, nausea, tiredness.
Seek medical attention right away if any of these severe side effects occur: severe allergic reactions (rash, hives, itching, difficulty breathing, tightness in the chest, swelling of the mouth, face, lips, or tongue, unusual hoarseness), chest pain, confusion, decrease in sexual ability, decreased or painful urination, depression, drowsiness, fainting, fast or irregular heartbeat, fever, chills, or persistent sore throat, increased or excessive sweating, muscle pain, tenderness, or cramps, red, swollen, blistered, or peeling skin, restlessness, seizures, severe or persistent dry mouth, shortness of breath, swelling of the arms or legs, unusual bruising or bleeding, unusual thirst, tiredness, weakness, vomiting, or yellowing of the skin or eyes.

This is not a complete list of all side effects that may occur. If you have questions about side effects caused by Telmisartan HCTZ tablets, contact your health care provider. Symptoms of an overdose may include confusion, fainting, fast, slow, or irregular heartbeat, severe or unusual dizziness.

Telmisartan HCTZ Overdosage

The following sysmptoms are observed in cases of Overdosage of Telmisartan HCTZ tablets (Generic Micardis HCT).

There is limited data available with regard to overdosage of Telmisartan in humans. The most likely symptoms of overdosage with telmisartan would be hypotension, dizziness and tachycardia. Bradycardia may occur from arasympathetic (vagal) stimulation. If symptomatic hypotension occurs, supportive treatment should be started. Telmisartan is not removed by hemodialysis.

The most common signs and symptoms observed in patients due to overdose of Hydrochlorothiazide are those caused by electrolyte depletion (hypokalemia, hypochloremia, hyponatremia) and dehydration resulting from excessive diuresis. If digitalis has also been administered to the patient, hypokalemia may accentuate cardiac arrhythmias.

Telmisartan Hydrochlorothiazide Tablets Contraindications

Telmisartan Hydrochlorothiazide tablets are contraindicated in patients with known hypersensitivity (e.g., anaphylaxis or angioedema) to Telmisartan, Hydrochlorothiazide, or any inactive ingredient of Sartel-H tablets (Generic Micardis HCT). Due to the presence of the hydrochlorothiazide component, Generic Micardis HCT is contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived drugs.

Warnings & Precautions When Using Telmisartan HCTZ Tablets

Do not use Generic Micardis HCT (Telmisartan HCTZ tablets) if you:
• are allergic to Telmisartan and Hydrochlorthiazide (the active ingredients) or to any other sulfonamide medicine (eg, sulfamethoxazole)
• have severe liver or kidney problems or are unable to urinate
• are taking dofetilide, ketanserin, or lithium

Some medical conditions may interact with this medicine. Tell your doctor if you have any medical conditions, especially if any of the following apply to you:
• if you are taking any, herbal preparation, or dietary supplement
• if you have allergies to medicines, foods, or other substances
• if you have ever had a severe reaction (e.g. swelling of the hands, face, lips, eyes, throat, or tongue; trouble swallowing or breathing; hoarseness)  after taking any other angiotensin II receptor blocker or an angiotensin converting enzyme (ACE) inhibitor (e.g. Captopril®)
• if you are dehydrated or have low blood volume
• if you have high or low blood electrolyte levels (e.g. sodium, chloride, potassium, magnesium, calcium)
• if you have asthma, diabetes, gout, heart problems (e.g. congestive heart failure), liver, gall bladder, or kidney problems, high blood cholesterol   or lipid levels, parathyroid gland problems, or systemic lupus erythematosus
• if you have recently had nerve surgery
• if you are taking another medicine for high blood pressure
• Female patients of childbearing age should be told about the consequences of exposure to Sartel-H tablets (Generic Micardis HCT) during pregnancy. Treatment options should be discussed with women planning to become pregnant. Patients must be informed to report pregnancies to their doctor immediately.
Some medicines may interact with this medicine. Tell your health care provider if you are taking any other medicines, especially any of the following:
• dofetilide or ketanserin because the risk of irregular heartbeat may be increased
• ACE inhibitors (eg, Lisinopril®, Ramipril®) because the risk of kidney problems may be increased
• adrenocorticotropic hormone (ACTH), barbiturates (e.g. Phenobarbital®), corticosteroids (e.g. Prednisone®), diuretics (e.g. Furosemide®), other medicines  for high blood pressure, or narcotic pain medicines (e.g. morphine) because they may increase the risk of this medicine side effects
• cholestyramine, colestipol, or nonsteroidal anti-inflammatory drugs (NSAIDs) (e.g. ibuprofen) because they may decrease this medicine effectiveness
• diazoxide, digoxin, lithium, nondepolarizing muscle relaxants (e.g. Tubocurarine®), potassium, or potassium-sparing diuretics (e.g. Spironolactone®) because the risk of their side effects may be increased by this medicine

Telmisartan HCTZ During Pregnancy

Telmisartan has been classified by the US FDA as Pregnancy Category D. Use of drugs such as Telmisartan HCTZ that act on the renin-angiotensin system during the second and third trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death. oligohydramnios may be associated with fetal lung hypoplasia and skeletal deformations.

Potential neonatal adverse effects include skull hypoplasia, anuria, hypotension, renal failure, and death. When pregnancy is confirmed or suspected, stop taking Generic Micardis HCT (Telmisartan HCTZ tablets) as soon as possible. These adverse outcomes are usually associated with use of these medications in the second and third trimester of pregnancy. Tell your doctor if you are pregnant or plan to become pregnant during treatment.

Telmisartan HCTZ Nursing Implications

It presently unknown if Telmisartan is excreted in human milk, but Telmisartan was proved to be present in the milk of lactating rats. Thiazides (including Hydrochlorothiazide) are known to appear in human milk. Because of the potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the medication, taking into account the importance of the medicine to the mother. Do not use this medication without telling your doctor if you are breast-feeding a baby.
