Generic Depakote ER (Dicorate ER tablets) is an anti-epileptic drug which is used for acute treatment of manic or mixed episodes associated with bipolar disorder, with or without psychotic features. Divalproex Extended Release tablets are also used as monotherapy and adjunctive therapy of complex partial seizures and simple and complex absence seizures and adjunctive therapy in patients with multiple seizure types that include absence seizures.
Dicorate ER tablets are also used for prophylaxis of migraine headaches. Dicorate ER tablets are manufactured by Sun Pharmaceutical Industries Ltd. in the strengths of 125 mg, 250 mg, 500 mg, 750 mg and 1000 mg.

Uses of Generic Depakote ER (Dicorate ER by Sun Pharma)

Generic Depakote ER (Dicorate ER tablets) is is a valproate used to treat various types of seizure disorders. It is also used to treat the manic phase of bipolar disorders (manic-depressive illness), and to prevent migraine headaches. The medicine affects chemicals in the body that may be involved in causing seizures.
Divalproex tablets may also be used together with other seizure medications.

Generic Depakote ER (Divalproex Extended Release Tablets) - Dosage

Generic Depakote ER (Divalproex Extended Release tablets) is a medicine intended for once daily oral administration. Dicorate ER tablets should be swallowed whole and should not be crushed or chewed.

Mania:
Generic Depakote ER tablets are administered orally. The recommended initial dose is 25 mg/kg/day given once daily. The dose should be increased as rapidly as possible to achieve the lowest therapeutic dose which produces the desired clinical effect or the desired range of plasma concentrations.

Epilepsy:
Complex Partial Seizures:
The dosage of adults patients and children 10 years of age or older should start Divalproex Extended Release tablets therapy at 10 to 15 mg/kg/day. The dosage should be increased by 5 to 10 mg/kg/week to achieve optimal clinical response.
Simple and Complex Absence Seizures:
The recommended starting dose of Divalproex Extended Release tablets is 15 mg/kg/day, increasing at one week intervals by 5 to 10 mg/kg/day until seizures are controlled or side effects prevent further increases. The maximum recommended dosage is 60 mg/kg/day. If the total daily dose is more than 250 mg, it should be given in divided doses.

Prophylaxis of Migraine:
Generic Depakote ER (Dicorate ER tablets) is indicated for prophylaxis of migraine headaches in adults. Dicorate ER tablets are administered orally. The recommended initial dose is 250 mg twice daily.

How to Use Generic Depakote ER (Divalproex Extended Release Tablets)

Take this medicine exactly as it was prescribed for you. Do not take the medication in larger amounts or take it for longer than recommended by your doctor. Drink plenty of water while you are taking this medicine. Your dose may need to be changed if you do not get enough fluids each day. Your liver function will need to be tested to be sure this medication is not causing harmful effects.

It is important that you do not miss any scheduled visits to your doctor. Do not stop taking this medicine even if you feel better. It is important to take this medication regularly to prevent seizures from recurring. Get your medicine refill before you run out of medicine completely.

Do not crush, chew, break, or open a delayed-release or extended-release tablet. Swallow the tablet whole. It is specially made to release medicine slowly in the body. Breaking or opening the tablet would cause too much of the drug to be released at one time.
Carry an ID card or wear a medical alert bracelet stating that you are taking this medication, in case of emergency. Any doctor, dentist, or emergency medical care provider who treats you should know that you are taking this medicine.

Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at the next regularly scheduled time. Do not take extra medicine to make up the missed dose.

Divalproex Side effects

The common side effects reported by 5% or more of Divalproex (Generic Depakote ER) treated patients during clinical trials were Nausea, Somnolence, Dizziness, Vomiting, Accidental Injury, Asthenia, Abdominal pain, Dyspepsia and Rash.

In rare cases, Divalproex Extended Release tablets has caused life-threatening liver failure, especially in children younger than 2 years old. Children of this age may be at even greater risk for liver problems if they use more than one seizure medication, if they have a metabolic disorder, or if they have a brain disease causing mental impairment (such as Creutzfeldt-Jacob disease, Huntington disease, multiple sclerosis, or a brain injury or infection). This medicine has also caused rare cases of life-threatening pancreatitis (inflammation of the pancreas). Pancreatitis can come on suddenly and symptoms may start even after you have been taking this medicine for several years.
 
Do not take Dicorate ER tablets if you have liver disease or a urea cycle disorder. If you have any of these other conditions, you may need a dose adjustment or special tests to safely take this medicine:
• a bleeding or blood clotting disorder
• a history of head injury, brain disorder, or coma
• a family history of a urea cycle disorder
• a family history of infant deaths with unknown cause
• HIV or CMV (cytomegalovirus) infection

You may have thoughts about suicide while taking Dicorate ER. Tell your doctor if you have new or worsening depression or suicidal thoughts during the first several months of treatment or whenever your dose is changed. Your family or other caregivers should also be alert to changes in your mood or symptoms. Your doctor will need to check you at regular visits. Do not miss any scheduled appointments.

Divalproex During Pregnancy

Divalproex has been classified by the US FDA as Pregnancy Category D for epilepsy and for manic episodes associated with bipolar disorder. It is also classified as Pregnancy Category X for prophylaxis of migraine headaches.

All pregnancies have a background risk of birth defects (about 3%), pregnancy loss (about 15%), or other adverse outcomes irrespective of drug exposure. Maternal valproate (Divalproex ER) use during pregnancy for any indication increases the risk of congenital malformations, particularly neural tube defects, but also malformations involving other body systems. Stop using this medication and tell your doctor right away if you become pregnant during treatment. This medicine may cause injury and even death to the unborn baby if you take it during the second and third trimesters of pregnancy. Do not use this medicine without your doctor's consent if you are pregnant.

Divalproex (Valproate) is excreted in human milk. Caution should be exercised when Divalproex ER tablets are administered to a nursing woman.Do not use this medication without telling your doctor if you are breast-feeding a baby.

Warnings & Precautions When Using Generic Depakote ER (Divalproex Extended Release Tablets)

The warnings and precautions to be taken when undergoing treatment with Generic Depakote ER (Dicorate ER tablets) is given below:

  • Hepatotoxicity:
    Hepatic failure resulting in fatalities has occurred in patients receiving valproate. Such incidents usually have occurred during the first six months of treatment. Serious or fatal hepatotoxicity may be preceded by non-specific symptoms such as malaise, weakness, lethargy, facial edema, anorexia, and vomiting.
  • Birth Defects:
    Generic Depakote ER can cause fetal harm when administered to a pregnant woman. Pregnancy registry data show that maternal valproate use can cause neural tube defects and other structural abnormalities.
  • Decreased IQ Following in utero Exposure:
    Generic Depakote ER can cause decreased IQ scores following in utero exposure. Published epidemiological studies have shown that children exposed to valproate in utero have lower cognitive test scores than children exposed in utero to either another antiepileptic drug or to no antiepileptic drugs.
  • Pancreatitis:
    Cases of life-threatening pancreatitis have been reported in both children and adults receiving Divalproex Extended Release tablets.
  • Urea Cycle Disorders:
    Generic Depakote ER is contraindicated in patients with known urea cycle disorders (UCD). Hyperammonemic encephalopathy, sometimes fatal, has been reported following initiation of valproate therapy in patients with urea cycle disorders
  • Suicidal Behavior and Ideation:
    Antiepileptic drugs (AEDs), including Divalproex Extended Release tablets, increase the risk of suicidal thoughts or behavior in patients taking these drugs for any indication.
  • Bleeding and Other Hematopoietic Disorders:
    Divalproex Extended Release tablets are associated with dose-related thrombocytopenia.
  • Hyperammonemia:
    Hyperammonemia has been reported in association with Divalproex Extended Release tablets therapy and may be present despite normal liver function tests.
  • Hyperammonemia and Encephalopathy Associated with Concomitant Topiramate Use:
    Simultaneous administration of topiramate and Generic Depakote ER has been associated with hyperammonemia with or without encephalopathy in patients who have tolerated either medicine alone.
  • Hypothermia:
    Hypothermia, defined as an unintentional drop in body core temperature to less than 35°C (95°F), has been reported in association with Divalproex Extended Release tablets therapy both in conjunction with and in the absence of hyperammonemia.
  • Interaction with Carbapenem Antibiotics:
    Carbapenem antibiotics may reduce serum Divalproex concentrations to subtherapeutic levels, resulting in loss of seizure control.
